Quite simple but must use below techniques for change folder name in iCloud Drive in iOS 9, User are confusing on different layout: List and Grid layout. Here I explained for both layouts.

For Grid view: change iCloud Drive folder/ File in iPhone – iOS 9
Step 1: Launch App, App home screen will be shown all folders.
For List View: Change folder name in iPhone’s iCloud drive
To see slide view option, just slide your finger from top to bottom and Tap on blue icon from right top side.
Step 1: Swipe right on folder, then you can see more and Delete option for particular folder.
Step 2: Tap on More > Rename Folder > Change name and Save for update.
This update also will be made in your iCloud web account.
